2016-2017 Enrollment Checklist

Welcome to the 2016-2017 school year! On this page, Gilman School parents will find the information necessary to enroll their son(s) for the upcoming school year.

Submit your contract by Friday, February 26, 2016

To re-enroll your son(s) for the 2016-2017 academic year, you must complete your contract in PCR.

Click here to complete your contract (you will navigate away from the Gilman website).

Please note:

  • The contract must have the signatures of both parents or guardians to be complete. In the case of divorced or separated families, the secondary contract must be completed.
  • The enrollment deposit must be paid before your contract can be submitted. You may pay the deposit using a credit card or bank account, or you may add the deposit amount to your current FACTS agreement.

arrange your tuition payment plan

Each family must select a tuition payment plan when completing the enrollment contract.

Families already enrolled in the FACTS program will AUTOMATICALLY be enrolled in the same plan for the 2017-2018
academic year. There are two options:
  1. Plan A: One payment in full on or before July 31, 2017.
  2. Plan B: Two or ten payments through FACTS Payment Program. Please note that the number of payments for re-enrolling students will remain as selected for the 2016-2017 academic year, but may be changed by contacting the Business Office. For new students, parents will select the number of payments when setting up the FACTS agreement.

Parents will receive an email confirmation from FACTS when the new enrollment is complete. The email will include the payment dates and amounts.

Please contact FACTS directly (866-441-4637) if you need to update your bank account information or if you have trouble accessing your FACTS account.


Tuition Refund Plan

Your financial obligation to the school is for the full annual tuition as stated in the school's enrollment contract. The school cannot refund tuition or cancel unpaid obligations if your child is forced to withdraw during the academic year. If your son withdraws, the Tuition Refund Plan will pay benefits (subject to the terms of the policy and the amount insured) to the school, which provides substantial assistance in meeting your financial obligation.

The tuition refund insurance fee will be billed to you in August and should be paid directly to Gilman upon receipt of your statement.

  • Lower School – Kindergarten, $242
  • Lower School – Pre-first and 1st grade, $298
  • Lower School – 2nd through 5th grade, $314
  • Middle School, $324
  • Upper School, $344

After you complete your contract in PCR, you will need to verify and update your household information. Please review each screen and click Save and Next at the bottom of each page.

Certain information is shared across family members, so you will only have to enter changes once. Once you finish the forms for your first child, click the link to complete forms for any additional children.

If you wish to change which parent is listed as Parent 1 and Parent 2, please contact Barbara Dorries at bdorries@gilman.edu.

Your son's annual health forms must be submitted through Magnus Health by July 1, 2016.

Instructions for Continuing Families

Your son's annual health forms must be submitted through Magnus Health by July 1, 2016.

Step One
:
Log onto the Gilman Parent Portal and click on the Magnus Health button under Useful Links on the left side of the page.


Step Two
:
Your son(s) Health Form To Do list will appear and you can begin to complete the information. Instructions for submitting documents like physicals, immunization record, and medication orders are on the Magnus website. PLEASE KEEP A COPY OF ALL DOCUMENTS.

These items will appear on your Health From To Do List:

  • Vital Health Record
    Review, revise and reauthorize the information you entered last year. Note: if you need to change your address or phone numbers, please return to the Gilman Parent Portal and do so through the Grades and Comments link. The new information will upload automatically to Magnus.
  • Consents
    Requires an electronic signature.
  • Physical Exam Form (Rolling Deadline)
    The date of the physical exam that we have on file for your son is noted here. We require that every boy turn in an updated physical exam form within one year and a month of the previous year’s exam. The extra month is to allow for insurance requirements. Example: John Smith had a physical on February 1, 2015. His next physical exam form is due by March 1, 2016. Helpful hint: most doctors book 2-3 months in advance for physicals, so call early for an appointment.
  • Over the Counter Medications
    Review, revise as needed and reauthorize.
  • Health Insurance Card
    If your son’s insurance has not changed, just reauthorize. If he has new insurance, upload a copy of the new card.
  • Special Health Conditions
    If your son does not need to take medication at school and/or does not have a medical condition like asthma, food allergies, diabetes or seizure disorder, simply click NO for the remaining questions and you are done.

    If your son does have a medical condition listed in this area, please check the appropriate boxes, have his doctor complete the required form(s) and upload, fax, or mail to Magnus. Blank forms are available for download on the Magnus site.

    Note: If the doctor completes the form for an inhaler, an epi-pen, seizure medication, or insulin/glucagon, you do not need to submit a prescription medication form as well.

    Students in the Middle and Upper Schools may self-carry and self-administer their inhalers and epi-pens with permission from parents, doctor and school nurse. If you would like your son to self-carry/self-administer, please click YES in response to the question in Magnus AND make sure the doctor signs the self-carry/self-administer portion of the appropriate form.

Step Two:
Your son(s) Health Form To Do list will appear and you can begin to complete the information. Instructions for submitting documents like physicals, immunization record, and medication orders are on the Magnus website. PLEASE KEEP A COPY OF ALL DOCUMENTS.

These items will appear on your son's Health Form To Do List:

    Please upload an up-to-date immunization record signed by a doctor.

Upper School parents whose sons who wish to park on campus must complete the Permission for Use of Campus Parking form, which is part of the school forms found in PCR.
