Download the Service Learning Commitment Form using the button below. You may either print and fill out the form handwritten, or you may fill it out digitally using Adobe Acrobat.

You, the student, must sign and date the Commitment Form yourself and then share it with and receive the dated signatures of:

• A parent/guardian
• The volunteer coordination of your proposed organization

These two signatories may sign and date the form handwritten (if you have printed it), or they may sign it digitally with Adobe Acrobat and return it to you for the other signatory to sign.

Your service learning project proposal will then be evaluated by the Assistant Head of School for Community, Inclusion, and Equity (CIE). If approved, you will receive the fourth and final signature and a confirmation from the Office of CIE authorizing you to begin your service learning project on the proposed date you listed.
